In a large pot, cook ground beef over medium heat until browned. Drain excess fat.
Add onion, garlic, and bell pepper. Cook until softened, about 5 minutes.
Stir in crushed tomatoes, tomato sauce, beef broth, Worcestershire sauce, Italian seasoning, paprika, salt, and pepper. Bring to a simmer.
Add uncooked macaroni. Cover and simmer for 15-20 minutes, stirring occasionally, until pasta is tender.
Remove from heat. Stir in cheddar cheese if using. Let sit 5 minutes before serving.
